UPDATED CDSP/Medical Clearance FOR STUDENTS IN HEALTH SCIENCES AND HUMAN SERVICES – September 16 2011


Programs: Cardiovascular Technology, Early Childhood Education and ECE Intensive, Occupational Therapist and Physiotherapist Assistant, Pharmacy, Diagnostic Cardiac Sonography, Personal Service Worker, Practical Nursing, Nursing, RPN TO BScN, Autism & Behavioural Science, Concurrent Disorders, Office Admin Medical 3rd Semester


Clearance Clinics


If you do not have a “clearance note” issued, you will not be able to attend the clinical/field component of your program. It takes minimum 2-3 weeks to complete the CDSP/Medical Clearance  screening process and often longer.




DURING A STRIKE, YOU MUST COMPLETE THIS PROCESS IN THE COMMUNITY such as at a walk in clinic or your family doctor’s office. For a list of walk in clinics, please see our website. Please note, that  walk in clinics and family physician offices may charge you a fee for this service. We do not have any control over this fee.


Download a Student Information Package from the Health Services website for details of medical clearance requirements. Take to your family physician or walk in clinic. When you have completed the requirements, you must get formally cleared by coming to a clearance clinic.


Please carefully note:


If you have completed all of your CDSP/Medical requirements in the community,  you can come to any clearance clinic and we will clear you or advise you regarding next steps. If you have questions or there have been complications in completing the requirements in the community you may also come to these clinics and we will advise you as to your next steps.


If you have files/lab results with us at IAHS Health Services please come to any clearance clinic listed below for IAHS campus.


If you have files/lab results at Fennell Health Services, you must either come to a Fennell clearance clinic or give us advanced notice that you will be coming to IAHS clinic so we have time to transfer your file.

NEW Requirement: Pertussis


VERY IMPORTANT: New and Returning students please note that vaccination against pertussis (with Adacel or Tdap) in the last 10 years is NEW a requirement of clearance that came into effect in August 2011. Many returning students who were cleared are now being denied access to clinical placements. You MUST check your immunization records and if you have not received Adacel it is STRONGLY RECOMMENDED that you get vaccinated at your family physician and walk in clinic immediately.


Do not wait for us to call you. When you get vaccinated, get proof of vaccination (yellow immunization record, doctors note, or computerized note) and bring to a clearance clinic ASAP or fax in the results to 905 528-0517. 


We will begin to call students who have not been vaccinated with Adacel in the last ten years, will be informed that they must get vaccinated and bring results (or fax results) to our offices within two weeks. Failure to do so will lead to having medical clearances revoked.


NOTE: HEP  B VACCINATIONS


If you paid for your Hep B vaccinations at Health Services, please be advised that you will not be able to get your second or third dose from Mohawk Health Services. You are still medically clear for clinical with one dose and may take the second  or third dose as soon as the strike is over and nursing staff return to work.
